Position Summary
The medical home team will work in conjunction with the primary care registered nurse (PCBH provider) to establish a comprehensive support and service system within the patient’s medical home. The position entails the provision of high-quality, population-based healthcare to patients, families, and communities, which is based on empirical evidence.
What Does the Role Do?
- Provide behavioral strategies and support to primary care patients with mental and physical health concerns, as well as identify, triage, and assess them.
- Obtain comprehensive training in the PCBH model to implement evidence-based treatment interventions and best practices.
- Provide care to patients of all ages, stages, and capacities, as well as families and groups.
- Consult, educate, and collaborate with community organizations, referral organizations, and internal and external healthcare teams.
Who Should Apply?
- Social employment, A Bachelor of Science in Nursing, or a Registered Psychiatric Nurse.
- Enrollment in a college that is authorized by the Health Professions Act.
- Current BLS (Basic Life Support) certification for medical professionals.
- A registered nurse or social worker must have a minimum of two to three years of experience in psychiatric emergency roles and/or community-based settings.
- Prior experience in a primary care setting or within a medical home/integrated care model is preferred.
- Knowledge of electronic medical records is advantageous.
- Experience in group development, facilitation, or instruction is a significant asset.
- Experience and training in cognitive behavioral therapy, health-related behavior change methods, and primary care behavioral health models are advantageous.
